Chattanooga CARES provides case management to 11 counties in southeast Tennessee including Hamilton, Bradley, McMinn, Polk, Grundy, Franklin, Marion, Rhea, Bledsoe, Sequatchie, and Meigs counties. |

The responsibilities of a CARES case manager are to assess each client at intake, develop a comprehensive service plan, act as a liaison for the referral of a client, monitor a client’s health and stability, and continually update and review the client’s service plan. All case managers have extensive experience in the field of social work. Our case managers work with hospitals, doctors, public assistance, child care providers, housing providers, food banks, mental health facilities, substance abuse professionals and other non-profit agencies to maximize a client’s ability to remain healthy, stabilized and functioning independently.

Case managers can assist those clients who have a need for housing through various programs including Housing Opportunities for People With AIDS (HOPWA), Shelter Care Plus and through working with the Housing Authority’s Section 8 program.

| CARES offers a variety of support groups and client education programs in an effort to meet the needs of our clients living with HIV/AIDS. Contact the Front Desk, or your Case Manager, for our current schedule.

CARES can arrange and sometimes provide transportation to clients for medical appointments, grocery shopping and other necessary appointments by coordinating with our substantial volunteer base. CARES also provides bus passes for those clients who qualify. Transportation, for rural county clients, can be arranged through SETHRA Rural Transportation.

Chattanooga CARES maintains a dry pantry to supply basic household supplies for clients who need them. We gladly accept donations in the form of certain items which you can view on our wish list.

Several volunteer organizations donate funds for nutritional supplements that are available to clients including Ensure, Nubasics, Boost and Advera.
|

Funding is available through the Ryan White CARE Act for several services. These funds are administered through a community organization funded by the State of Tennessee. Clients who cannot afford certain medications such as protease inhibitors and other drugs that assist with living with HIV can access the AIDS Drug Assistance Program (ADAP). Medical and dental care is available for those persons who are not on insurance or TennCare and meet certain financial criteria. Ryan White funds are also available for private insurance continuation through the AIDS Insurance Continuation Program (AICP).

Chattanooga CARES has a limited amount of financial resources through several grants to provide emergency financial assistance for housing, utilities, food and medical needs. Each request is reviewed on a case by case method, and clients must meet certain requirements to qualify.
